Cardiac Rehab Program

Our “Hearts in Motion” program provides medically supervised cardiac rehabilitation services to patients who have experienced a cardiac event within the last two years. The program is located at the South Bruce Grey Health Centre (Kincardine Site), and is directed by the Kincardine Family Health Team in accordance with clinical standards of practice, established by St. Mary’s General Hospital (Kitchener, Ontario).

Patients of cardiac rehabilitation typically complete the program in 16 weeks and will then be discharged, and encouraged to pursue independent activities in the community to support their health.

ELIGIBILITY Patients who have experienced a cardiac event within the last two years.

REQUIREMENTS Patients can discuss referral with their Family Physician, but can also self-refer.

Chronic Disease

Are you at risk for, or have you been diagnosed with, a chronic disease? The BAFHT and KFHT offer patient education, self-management support, and routine monitoring to help you with the prevention, early diagnosis and management of chronic diseases. If your Primary Care provider is seeing emerging signs of cardiovascular disease, diabetes, metabolic syndrome, or another chronic disease, we can help you focus on developing and maintaining lifestyle behaviours that keep you healthy and active longer.

ELIGIBILITY Individual appointments and community and group educational sessions available to all members of the public.

CHANGE Program

If you are at risk for, or have been diagnosed with, Metabolic Syndrome, the Canadian Health Advanced by Nutrition and Graded Exercise (CHANGE) Program can help. The CHANGE Program runs for 1 year to offer you support to make long-term, preventative changes. A personalized nutrition and fitness program will be created to target your risk factors for heart disease, stroke, and diabetes and reduce the need for medication. You will meet quarterly with a Nurse Practitioner, and weekly during the first 3 months with our Registered Dietitian/Kinesiologist.

ELIGIBILITY For rostered patients only.

Community Outreach

Would you like a BAFHT or KFHT team member to visit your group/organization/workplace to provide an education session about Chronic Disease management? We can work with you to provide a session that is both informative and relevant to the interests of the group.

Chronic Pain

One 1.5 hour session, utilizing non-pharmacological and lifestyle strategies to assist patients with learning to live well with chronic pain. Follow-up appointments are encouraged as well.

Foot Care

Are you in need of foot care? The BAFHT Chiropodist can assess and treat acute and chronic foot conditions that may arise as you age or as a result of diseases such as gout and diabetes. The BAFHT Foot Care Nurse can provide basic foot care. Both offer education to help you self-manage your foot health.

Kinesiology

Interested in improving your well-being through physical activity? If you have a mobility disorder, a strength and balance issue, an acute musculoskeletal injury, a chronic disease or osteoporosis, we can help. The BAFHT and KFHT Kinesiologists offer one-on-one consultations and physical activity programs to help you move better, live longer and be healthier. They can assess your current level of function, develop a plan to improve your well-being, and set you up for long-term health benefits.

Flex, Stretch and Balance 50+

As we age, we naturally begin to lose our flexibility. Improve your body’s range of motion, relieve stress and make you feel better overall. An informative exercise program for adults 50+. A drop-in group exercise program offered for 6 weeks with a focus on flexibility, balance, strength and general conditioning.

Stride Alive

Stride Alive is a low-and-slow resistance program designed for anyone looking to increase their physical activity level.

Stride Alive takes place every Tuesday and Thursday morning from 9:00 – 10:15 at the Davidson Centre Indoor Walking Track in Kincardine. This group exercise program increases activity while building life long friendships.

Be prepared to work up a sweat as each session includes walking, balance training, resistance training and flexibilty.

ELIGIBILITY For all members of the public.

REQUIREMENTS Patients can discuss referral with their Family Physician, but can also self-refer. New participants will meet with an RN at the Family Health Team for an in-take assessment prior to joining the group.

Boosting Balance

This is a 4-week exercise and education program for seniors 65 and above.

This program includes:

  • A balance assessment at the beginning and end of the program so you can track your progress
  • Instructive exercises you can do at home to promote strength, flexibility, balance, and endurance now and for life
  • Strategies to maintain health and prevent injuries at home and in the community

ELIGIBILITY For all members of the public.

Team UNBREAKABLE

Running and physical activity have well-known physical and mental benefits. Team UNBREAKABLE, a Cameron Helps program, is a physical-health-for-mental-health-program for teenagers, started in Toronto and implemented locally by the BAFHT and KFHT teams. Our Team UNBREAKABLE program is an annual event that provides an opportunity for teenagers to train with FHT staff, at a comfortable and achievable pace, in preparation for a 5-km run. A weekly presentation on mental and physical wellness is provided and social inclusion is encouraged.

ELIGIBILITY Offered in collaboration with local school boards.

Memory Clinic

Having memory issues? Help maintain independent living through the early detection and treatment of memory problems. The goal of Memory Clinic is to identify cognitive issues and to provide treatment and support options to help manage memory problems. Memory Clinic testing includes an assessment of your ability to safely drive. The BAFHT and KFHT Memory Clinic Teams include a Family Physician, a Nurse, a Social Worker and a Pharmacist, and includes cognition testing, a medication review, and a caregiver burden assessment.

Mental Health

Are life stressors or emotional issues getting in your way of living life well? Our Social Workers and Counsellors offer counselling services for anxiety, depression, substance abuse, relationship stress, mood disorders and other life stressors. We offer counselling for individuals, couples and families, and work with you to establish goals, develop strategies, and work toward individual and relationship wellness.

Neuro Rehab Program

This program runs in 10 week sessions, held upstairs at Trillium Court. After completion of the program patients will be discharged and encouraged to pursue independent activities in the community to support their progress. Patients diagnosed with a neurological condition (excluding ABI/Concussion) are good candidates for this program. Patients must be able to walk 10 metres with or without a walking aid, but unassisted by another person.

ELIGIBILITY Available to all members of the public with a diagnosed neurological condition.

REQUIREMENTS Patients can discuss referral with their Family Physician or self-refer. New participants will meet with an RN or OT at the Family Health Team for an in-take prior to joining group.

Nutrition

Thinking about improving your nutrition? The BAFHT and KFHT Dietitians provide clinical nutritional assessments and interventions for a variety of nutrition-related conditions including diabetes, heart disease, celiac disease, IBS, IBD, electrolyte disturbances, micronutrient deficiencies and weight management – through individual appointments and group workshops. Nutritional education is provided on a variety of topics, including healthy family eating, maternal nutrition, introduction of solids, picky eating and sports nutrition. Individual assessments are for rostered patients. Community outreach sessions listed below are available, as advertised or upon request.

Craving Change

A how-to workshop for changing your relationship with food. Understand why you eat the way you do, learn to comfort yourself without food, change your thinking, and change your eating. Four sessions, 2 hours each.

Occupational Therapy

An individual assessment focused on improving or maintaining function to live independently. Interventions may include, fall prevention, energy conservation, home assessments and modification, assistive devices, health system navigation and caregiver education. The Kincardine Family Health Team’s Occupational Therapists offers a wide variety of programs to ensure better care for patients. These programs include:

Pre/Post Natal Support

Pregnant? Recently given birth? Hope to breastfeed or are breastfeeding? The BAFHT and KFHT offer support and information to help you adjust to parenthood and healthy family living – before and after your baby arrives! We offer prenatal classes in two convenient formats and, once baby is here, we’re available for postpartum support, information, help and answers for breastfeeding questions, concerns or difficulties. Eligibility for these services varies; please see below.

Prenatal Care

The BAFHT and KFHT Prenatal Classes are facilitated by a Registered Nurse and a Kinesiologist, and offer information on keeping healthy during pregnancy, prenatal and postnatal exercises, labour and delivery, breastfeeding, infant care and behavior, safety, adjustment to parenthood and healthy family living. Choose from a 1-day condensed Saturday Workshop or a Four-Session Series Workshop offered one evening a week for 4 weeks.

ELIGIBILITY For all expectant mothers and their partners/support persons.

Postpartum Support

If you’ve recently given birth, the 6-week Postpartum Core/Floor Restore Exercise Program can help you recover from childbirth, restore and retrain pelvic floor and core muscles, strengthen large muscle groups, and safely return to exercise. This program is appropriate for women experiencing postpartum bladder leakage, back pain, pelvic pain or pain with intercourse.

ELIGIBILITY For all new mothers.

Early Years Lactation Workshop

Breastfeeding information and support sessions provided by a Certified Lactation Consultant at the Ontario Early Years Centre in Walkerton.

Breastfeeding Education and Support

Are you an expectant or new nursing mother? If you have any questions before baby arrives, are unable to get your baby latched on well, your nipples become sore, cracked or bleeding, the engorgement does not go away after 48 hours or if you are not sure that breastfeeding is going well, a Certified Lactation Consultant can provide breastfeeding education and support.

ELIGIBILITY Individual appointments are available to all expectant and new mothers.

REQUIREMENTS No referral required, but please register in advance.

Primary Care

Working in collaboration with the BAFHT and KFHT physicians, the Nurse Practitioners provide primary care visits focused on wellness, prevention, chronic disease management, diagnosis and treatment.

Telemedicine Program

Working in collaboration with the BAFHT physicians and the Telemedicine Coordinator, the Telemedicine Nurse Practitioner provides primary care visits that can include: comprehensive health assessment; monitoring and management of chronic diseases such as diabetes, arthritis, asthma and heart disease; prescribing and monitoring of medication; immunizations; annual physical exams; episodic illness care; referral to specialists and other human services; health teaching; smoking cessation; injury prevention; falls prevention assessment and information for older persons; and consultation and service coordination.

ELIGIBILITY Individuals who require a primary care provider and/or connection with CMHA-GB. Program services are available to individuals aged 18 years or older and who have either a serious mental illness or addiction issue (excluding tobacco as a primary addiction)

REQUIREMENTS Referral from CMHA counsellors/workers, ER physicians and primary care providers.

Nurse Practitioner Same-Day Appointment Clinic

The Kincardine Family Health Team offers a Nurse Practitioner Same-Day Appointment Clinic. Please note that this is not a walk-in clinic.

Patients are required to first contact their family physician in an attempt to schedule an appointment. Often times, you may have to leave a voicemail if your physician’s office is not yet open. If your family physician’s office does not have availablity, you can call the Kincardine Family Health Team for an appointment. There are a limited number of same-day appointments available and they fill quickly. Not all calls will receive a same day appointment. Patients may be asked to call back the next day if, again, they are unable to connect with their family physician. This is a same-day appointment clinic and appointments cannot be scheduled in advance.

We do not renew prescriptions and do not prescribe narcotics.

In case of emergency, please call 911 or visit your nearest emergency department.

ELIGIBILITY Individual appointments for rostered and non-rostered patients. No referral required. Strictly limited to only one concern per visit and non-chronic issues. Patient must first attempt to book with their Physician’s office prior to calling the Kincardine Family Health Team for an appointment.

REQUIREMENTS Rostered and non-rostered patients; same day clinic. Acute issues only.

LOCATIONS Available at the Kincardine FHT clinic from Monday to Thursday 8:30 am to 4:30 pm.

Respiratory Health

Having difficulty breathing? If you are living with respiratory disease, our goal is to improve and prolong your quality of life, and to decrease hospital and emergency-room visits through ongoing support. Our Respiratory Therapist can monitor, evaluate, treat and counsel patients with respiratory and cardio-respiratory disorders, such as chronic obstructive pulmonary disease (COPD), asthma, lung cancer, interstitial lung disease, cystic fibrosis, sarcoidosis or other respiratory diseases. We offer referrals for Spirometry testing, provide assistance with inhaler technique, and promote a healthy, active lifestyle through patient education and exercise classes.

COPD Activation Program

For individuals with mild to moderate COPD, our Activation Program may improve exercise tolerance, decrease disease progression, enhance your understanding of COPD and improve your ability to self-manage your disease. This self-management workshop series includes specific COPD education, medically-supervised exercise sessions, and the development of an ongoing exercise program to continue at home. Led by a team that includes a Registered Respiratory Therapist, Registered Kinesiologist and Registered Nurse.  This program includes 6 combined sessions of education and monitored exercise.

ELIGIBILITY Available to all members of the public.

REQUIREMENTS Referral from Family Physician or Nurse Practitioner preferred but not required.

Smoking Cessation

Are you interested in quitting smoking or reducing your tobacco intake? Quitting smoking or reducing your tobacco intake is the single most important thing you can do to improve your health and quality of life. We can help! We provide individual counselling and support to help you meet your smoking cessation goals. Through the Smoking Treatment for Ontario Patients (STOP) Program sponsored by the Centre for Addiction and Mental Health and the Ministry of Health Promotion and Sport, a BAFHT Nurse Educator can provide you with free nicotine replacement therapy and support to help you stop smoking cigarettes or reduce your tobacco intake.

Do you require assistance navigating the healthcare system or accessing local programs and services? Our System Navigators can assist with identifying local health and social services that might be of help, and coordinating connections to these services. We recognize how access to proper housing, healthy food, and transportation all contribute to good health. We have lots of great programs and services available in our region, but knowing when and how to access them can be confusing. We can help!
